Billing / Insurance FAQ

Q: When is payment due on services?

A: Full payment is due at the time of service unless we participate with your insurance carrier.

Q: What types of payment do you accept?

A: We accept Cash, Checks, Visa, MasterCard and Discover. We do offer an extended payment plan with prior credit approval.

Q: What Insurance plans do you participate with?

A: We have negotiated the following plans

  • Blue Cross / Blue Shield
  • Medicare
  • Cigna
  • Aetna - PPO
  • United Health Care
  • Beech Street
  • Florida First
  • Southwest Regional Medical Center - PHO
  • LEE Independent Physicians Association
  • and most others

Q: What other things should I know regarding insurance?

A: We cannot bill your insurance company unless you give us your insurance information and an original claim form. Your insurance policy is a contract between you and your insurance company. We are not a party to that contract.

In the event we do not accept assignment of benefits we require that you be pre-approved on our extended payment plan or provide a credit card with authorization to bill that account for the balance.

If your insurance company has not paid the bill within 45 days , the balance will be automatically be transferred to your credit card or the extended payment plan.

Please be aware that some, and perhaps all, of the services provided may be non-covered services and not considered reasonable and necessary under the Medicare Program and/or other medical insurance.
