Q: When is payment due on services?

A: If you are a self-pay patient payment is due in full at the time of service. If we are participating with your insurance company your co-pay, co-insurance, deductible or non-covered services are due in full at the time of service.

 

Q: What types of payment do you accept?

A: We accept Cash, Checks, Visa, MasterCard and Discover.

 

Q: What Insurance plans do you participate with?

A: We accept Medicare, BCBS of Fl and Most major insurance companies.

 

Q: What other things should I know regarding insurance?

A: We cannot bill your insurance company unless you give us your correct insurance information. It is your responsibility to keep our office updated with any changes. Please be aware that some, and perhaps all, of the services provided may be non-covered services and not considered reasonable and necessary under the Medicare Program and/or other medical insurance.
